Terry Lee Hazelwood, 69, formerly of Fort Smith, Arkansas passed away on Monday, December 25, 2017 in a Perry, Oklahoma nursing home. He was born on October 5, 1948 in Oklahoma City, Oklahoma. He was a graduate of Bolsa Grande High School in Westminster, California and a retired forklift operator. He was a member of Haven Heights Baptist Church in Fort Smith, Arkansas.
Funeral services are 1:00PM, Saturday, December 30, 2017 at Ocker-Putman Funeral Home Chapel of Fort Smith with burial to follow in Roselawn Cemetery.
The family will visit with friends from 5-7PM on Friday at the funeral home.
He is survived by two brothers, Bill Hazelwood of Lamont, Oklahoma and Ron Hazelwood of Anaheim, California and several nieces, nephews and friends.
